At times, members of the public may incorrectly direct privacy complaints to a particular program or agency. The following templates provide sample language for a variety of circumstances that may arise in which it is appropriate to acknowledge receipt of a complaint and redirect an individual elsewhere.
The examples below are provided for informational purposes only. State Health Departments should edit these samples to the circumstances of each case, as well as to match their communication style and meet any legal requirements or agency expectations.
